Renew prescriptions

Contact your pharmacy to fax your prescription request to our clinic at fax number 416-603-5448.

Important information about prescription renewals:

  • We respond to fax requests within 2-3 business days

  • If you need medication more urgently speak with your pharmacist about bridging your medication

  • Do not call the clinic for an update on your prescription renewal

  • We do not renew controlled substances or narcotics

Submit a photo/document

This form is only for non-urgent submissions of photos or documents to your provider. Please do not send photos for new medical concerns, book an appointment with your provider first. 

Important information about submitting your document/ form or photo:

  • Documents/ Forms: you must complete your section and/ or personal informaton BEFORE submitting your document or form. If your form is incomplete, we will not be able to upload it to your chart and this will delay the process. 

  • Photos: include a message about your photo before submitting it. 

Accepted file formats include JPEG, PNG, PDF, text, and standard media files, with a maximum size of 10MB per file. If your files exceed 10MB, please submit them in multiple forms.

We will notify your provider of your submission and add it to your chart. For urgent concerns, please call the clinic at 416-603-5888.

511 Bathurst – Stop at Nassau Street, between Dundas and College Streets

Parking
Underground paid public parking is available in the RioCan lot at Bathurst College Centre. This is accessible through Bathurst Street if you are going south. See here for details and rates.

Street parking is restricted on the streets around our clinic and Toronto Western Hospital. Please read parking signs carefully. 

Leonard Avenue parking lot: 53 Leonard Avenue, M5T 2S8

  • Parking is available in the lot on Leonard Avenue (enter off Leonard).
  • Please note: this parking area has 8 accessibility parking spots.
  • This parking lot is always open.
  • Cash, Visa and Mastercard are accepted.

The Bathurst Street/Nassau Street parking lot permanently closed on February 9, 2024 for the construction of the new surgical and patient tower. Learn more about the new surgical and patient tower.

Book an appointment with your primary care provider

For online booking with your primary care provider, you may:

  • Book a 15-minute appointment, in-clinic for new concerns
  • Book a 15-minute appointment, in-clinic for follow-up visits

**We are not offering online booking for appointments such physicals, well-baby visits or procedures.  If you choose to book for this type of visit, the request will be cancelled which will delay the next available appointment for you.**

  • You need an appointment for an urgent issue in the next 2-3 days
  • You need an appointment that is longer than 15 minutes (like physicals or a well-baby visits)
  • You do not have an OHIP number
  • You need an appointment with an allied health provider
